What Is Complex Regional Pain Syndrome?
Complex Regional Pain Syndrome is a neurological condition that can occur anywhere in the body but is most prevalent in the extremities. Its most common symptom is severe pain in the affected limb that seems disproportionate to the injury or circumstances. For example, people living with CRPS may experience extreme pain spontaneously or in response to a pinch or even a gentle touch. Other symptoms may include sensory changes as well as changes in skin color, temperature, and texture.
The onset of CRPS often, but not always, follows an injury or trauma to the affected body part. The two primary types of CRPS are:
- Type 1 CRPS (CRPS-1) – This form of the condition occurs after an illness or injury that isn’t directly associated with any known nerve damage. Also known as Reflex Sympathetic Dystrophy, it accounts for the vast majority of CRPS cases.
- Type 2 CRPS (CRPS-2) – This form of the condition occurs after an illness or injury that is directly associated with known nerve damage.
Complex Regional Pain Syndrome can result in acute (short-term) or chronic (long-term) pain and discomfort. Although there is no known cure for the condition, it often resolves over time and can be effectively managed using a spectrum of different treatment options.
Symptoms Of Complex Regional Pain Syndrome
The symptoms of Complex Regional Pain Syndrome may vary from patient to patient and can also evolve over time. They may include one or more of the following:
- Extremity pain
- Pain may be constant or intermittent and is often felt deep within the affected extremity. At Resolve, our doctors have often heard the quality of pain associated with CRPS described as burning, stinging, or tearing.
- Increased sensitivity to painful stimuli and/or non-painful stimuli
- An otherwise insignificant pinch or poke may be perceived as unbearably painful. In some cases, a soft touch or gentle pressure may also trigger an extreme pain response.
- Swollen and/or stiff joints
- Changes in skin color, temperature, or texture
- Skin that is pale, red, or blotchy
- Skin that is shiny or uncharacteristically sweaty
- Skin that is warmer or cooler than normal
- Swelling and/or redness in the affected area
- Decreased motor ability in the affected area
- Rapid hair/nail growth or no hair/nail growth
What Causes Complex Regional Pain Syndrome?
The exact causes of Complex Regional Pain Syndrome are not fully understood, but the condition is associated with over-reactive pain signals that result from abnormal function in the central or peripheral nervous system. Cases of CRPS that develop without an injury (Type 1) may be more likely to occur in patients who:
- Have poor nerve health
- Have existing immune system issues, such as inflammatory or autoimmune diseases
- Have family members with CRPS (more research is needed in this area)
Cases of CRPS that occur as the result of nerve damage (Type 2) are often associated with the following:
- Bone fractures
- Sprains and strains
- Burns
- Cuts
- Immobilization, such as being in a cast
- Surgery
Mental health issues such as anxiety and depression may also increase the risk of developing CRPS as can smoking and conditions associated with poor circulation.
Diagnosis And Treatment
At Resolve Pain and Spine, we are dedicated to uncovering hidden causes of chronic pain and will take all the time necessary to properly diagnose your symptoms. If CRPS is suspected after an in-depth review of your medical history and a physical examination, other diagnostic tests may be ordered to provide better visualization and rule out other possible causes of your pain and discomfort. These diagnostic tests could include an X-ray, CT scan, MRI, EMG, bone scan, sweat production test, and more.
There is no one-size-fits-all treatment for CRPS. Our team will work with you to develop a plan tailored to your needs that may include one or more of the following treatment options:
- Acupuncture
- Trigger point injections
- Nerve blocks
- Spinal cord stimulation
- Peripheral nerve stimulation
- Non-opioid pain treatment
